1.       An investigation is the examination, study, searching, tracking, and gathering of factual information that answers questions or solves problems.
2.       When should you conduct an investigation? Examples include: safety violations, on-the-job injuries, harassment and more.
3.       Use a checklist to help keep you organized during the investigation; who will you interview, what questions will you ask, do you need pictures, are all things to consider.
4.       A good investigator is professional, a good listener, maintains objectivity and is honest and truthful.
5.       Investigation records should be clear, legible, completed and include details that illustrate the root cause.
