North Carolina Cooperative Extension
Economic Impact Survey
 

North Carolina Cooperative Extension partners with communities to deliver education and technology that enrich the lives, land, and economy of North Carolina. Offices in all 100 counties and with the Eastern Band of Cherokee Indians, allow Cooperative Extension to provide relevant, science-based information to North Carolina citizens. The purpose of this survey is to determine the impact of North Carolina State University’s Cooperative Extension programs on the lives and livelihoods of North Carolina citizens. The survey should be completed by individuals (age 18 or older) or representatives of businesses or local/county agencies who participated in Cooperative Extension programs or had direct interaction with Cooperative Extension during 2013. All answers given should be for the 2013 calendar year.

Survey results will be compiled by the Center for Urban Affairs and Community Services at NC State University.  All answers are completely confidential and will be grouped for analysis purposes.  If you have questions about the survey, please contact Mike Yoder at 919-513-3509 or at mjyoder@ncsu.edu.

Instructions for Completing the Survey:
• Answer all survey questions. Select Don’t Know/Not Applicable when appropriate.
• Answer all questions with respect to your use of Cooperative Extension in 2013.
• Business Respondents: The survey should be completed by someone who can provide data on the economic impact of services received through Cooperative Extension. In most cases this will be an owner, CEO, CFO, or business manager.
• Please click the 'Back' button on the bottom of the survey to go back and change an answer. Please use the 'Proceed to next section' button to go forward through the survey.
