IMPORTANT NOTES:

1. Please remember to complete the Application in a pen color OTHER than black (if mailing original).
2. If you use a P O Box address, you must also provide the corresponding street address.
3. BE SURE TO READ AND FOLLOW INSTRUCTIONS TO SEND FORM TO APPROPRIATE ADDRESS. 
4. Illegible and/or incomplete forms cannot be processed.
5. Information on these forms cannot be altered, corrected, or added on to by USDA personnel.
6. All Information on the Application is subject to verification
7. All previous accreditations that have not submitted the new 1-36A (Dec 2009) form will be expired. Currently accredited veterinarians that do not complete this process will no longer be accredited after 08/02/2010.
